Privacy Policy

Effective Date:  July 23, 2023

Life Cycle Women’s Health, LLC., (“Life Cycle Women’s Health,” “we,” “us,” or “our”) recognizes the importance of protecting the privacy of your personal information, and we have prepared this Privacy Policy (“Policy”) to provide you with important information about the privacy practices applicable to www.lifecycleforwomen.com and any website (collectively, the “Sites”) or service(“Service”) that links to or refers to this Privacy Policy.  By accessing the Sites or using the Service, you agree to be bound by this Policy.  Each time you use any Site or Service, the current version of this Policy will apply.  Accordingly, you should check the date of the Policy (which appears at the top) and review any changes since you last reviewed the Policy. This Policy applies to Life Cycle Women’s Health and AB Telemedicine Care, PLLC, an independent, physician-owned entity to which we serve as manager.

I. COLLECTION OF INFORMATION

We may collect the following kinds of information when you use the Services:

Information you provide directly to us. For certain activities, such as when you register, use our health services, subscribe to our alerts, or contact us directly, we may collect some or all of the following types of information:

  • Contact information, such as your full name, email address, mobile phone number, and address;
  • Username and password;
  • Payment information, such as your credit card number, expiration date, and credit card security code;

  • Any other information you voluntarily provide to us.

Information we collect automatically. We may collect certain information automatically when you use our Services, such as your Internet protocol (IP) address, device and advertising identifiers, browser type, operating system, Internet service provider, pages that you visit before and after using the Services, the date and time of your visit, information about the links you click and pages you view within the Services, and other standard server log information.  We may also collect certain location information when you use our Services, such as your computer’s IP address, your mobile device’s GPS signal, or information about nearby WiFi access points.

We may use cookies, pixel tags, local shared objects, and similar technologies to automatically collect this information.  Cookies are small bits of information that are stored by your computer’s web browser.  Pixel tags are very small images or small pieces of data embedded in images, also known as “web beacons” or “clear GIFs,” that can recognize cookies, the time and date a page is viewed, a description of the page where the pixel tag is placed, and similar information from your computer or device.  By using the Services, you consent to our use of cookies and similar technologies.

We may also collect technical data to address and fix technical problems and improve our Services.

Information we obtain from your health care providers and other sources. In connection with the Services, we may collect medical and health information from you or your health care providers.  This may include information about your diagnosis, previous treatments, general health, laboratory and pathology test results and reports, social histories, any family history of illness.  Life Cycle Women’s Health is not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191, and its related regulations and amendments from time to time(collectively, “HIPAA”), but health care providers involved in the delivery of the Services may be “covered entities” under HIPAA, and we may in some cases be a “business associate” of a health care provider. HIPAA does not necessarily apply to us or to a provider due to the mere fact that health information is involved in our interactions, and HIPAA may not apply to your transactions or communications with us or the providers. Any medical or health information that you provide that is subject to specific protections under applicable state laws (collectively, with “protected health information,” as defined under HIPAA), will be used and disclosed only in accordance with such applicable laws.
